FM Stakeholder Questionnaire

This questionnaire has been developed to assist with gathering input and feedback from forest certification stakeholders. The questionnaire was developed and is administered by SCS Global Services (SCS). More information about SCS is available at www.SCSglobalServices.com.

INSTRUCTIONS:
There are 10 specific questions as well as space to provide general comments. This questionnaire should take less than 15 minutes to complete.

While responses may be provided anonymously, respondents have the option to provide contact information and requesting follow-up contact from a representative of the audit team. Contact information will only be used for the purposes of forest certification stakeholder consultation. Personal information will not be associated with any comments provided. Contact information will not be sold or shared in any other way unless required by law.
