Creating a Major Regional Health System for Southwest York Region
 

1. What city or town do you live in?

2. The hospital’s vision is to create a major regional system which includes two full-service hospitals - building a new hospital in Vaughan and further development of York Central Hospital. How satisfied are you that our vision will adequately meet the future health care needs of Southwest York Region?

3. Creating a new brand which includes a new name and visual identity is an important component of our major regional health system. Although we are in the early stages of our branding process, we envision that the two hospitals will be under one overarching brand. How involved would you like to be in creating a new identity for our health system?

4. The proposed two-hospital model will offer Emergency, Medical/Surgery Beds, Intensive Care Unit, Operating/Ambulatory focus and Clinical Support Services at both hospital sites. Do you feel the proposed services address the following health care priorities of our community?

5. Integrated Stroke Care, Chronic Kidney Disease and a Seniors/Chronic Centre of Excellence will also be offered as regional programs. With these services, do you agree that our major regional health care system will address the health care needs of Southwest York Region?

6. Our development project vision is Connecting you with tomorrow’s health care. This reflects a health system that is patient centred, focused on health promotion and prevention, incorporates technology in the delivery of care, and uses creative facility design to improve health and well-being of patients. As our project vision helps to guide our project planning, do you feel we are moving in the right direction with our project planning?

7. Our proposed site planning for our two hospitals includes using innovative designs to promote a healthy community (i.e. green space, walking trails), providing access to the hospital sites through public transit infrastructure and adequate entry and exit routes. What other aspects should we consider as we continue planning for our two hospital health care system?
