This is a questionnaire to help Missouri Foundation for Health (MFH) become more informed about the previous experiences community-based organizations have had in applying for federal funding, and to determine the type of support needed to help those organizations become more successful in capturing federal funding.
Please encourage other colleagues across Missouri to complete this survey. More than one person at your organization may fill out this survey. This will help us have multiple perspectives. It is important for MFH staff to hear your opinions in order that we may collectively ensure that community health and prevention programs in Missouri have adequate support through federal dollars, particularly those authorized under the Affordable Care Act (ACA).
There are no “right” or “wrong” answers. Your specific responses will not be shared outside MFH. Note: Respondents may skip questions they do not wish to answer.
