HealthInfoNet workshops: helping to ‘close the gap' in Indigenous health – Information letter post web-based questionnaire

Aims
The HealthInfoNet has provided workshops to train people working in Indigenous health on the use of the Australian Indigenous HealthInfoNet website.

Evaluation
This questionnaire is designed to find out about the use of the HealthInfoNet website by Western Australian health practitioners and what training they may have received in its use. We are only surveying people who live in WA because we are testing a new training program in that state.
There are 5 questions to answer. The questionnaire should take only a few minutes to complete.
We are undertaking this research to find out more about how people use our website and what we can do to improve the service we offer, including providing better training.

Confidentiality
The information you provide will only be used to evaluate the usefulness of the workshop training in supporting people working in Indigenous health. Participants can be assured of their privacy; all identifying information will be kept in separate lockable filing cabinets at the HealthInfoNet workplace. The project coordinator will have access to the data. Information will be kept securely for five years and then all paper forms will be shredded and all electronic files will be deleted.

Findings
Findings will be published on the HealthInfoNet website. Presentations will also be made to relevant stakeholder groups and at professional conferences. Longer term, findings will be published in appropriate professional and scientific journals.

Voluntary participation
Your participation in the post survey is completely voluntary and you may choose not to participate. If you give consent to participate you are free to withdraw at any time.
