1. Welcome to the Coach Darcy "You and Your Workplace" Survey!

Hi, and thanks for your willingness share your thoughts with me. This will be short, sweet, and hopefully, fun!

As my thanks for your time, I'll be drawing THREE names from all the completed surveys as of July 31, and sending them each a copy of Zappos.com CEO Tony Hsieh's new bestseller, Delivering Happiness. The stories in this fun, easy-to-read book are proof that we can create great workplaces that still let us have great lives!

(Just a note: all of the survey content will be kept completely separate and confidential from the names included in the drawing and will not be used for any other purpose. You can still complete the survey and not share your name, however.)
