DEMENTIA ALLIANCE Alzheimer Societies of Brant/Haldimand Norfolk/Hamilton Halton
Strategic Planning Process
Effective April 1, 2010, the Alzheimer Societies of Brant, Haldimand Norfolk and Hamilton Halton integrated their delivery of services to those affected by dementia under a new corporation called Dementia Alliance. This corporation is composed of three board members from each of the local Societies, and is meant to create efficiencies and ensure the best use of our financial and human resources to meet the Rising Tide of persons affected by dementia across our communities. Our local Societies continue to exist and act as the local face of our services and assist in our fundraising and public awareness efforts.
As part of its new mandate, Dementia Alliance has initiated a project to establish a three-year strategic plan identifying future directions and priorities. Part of this process involves obtaining the views of Board and staff members and our key external stakeholders about the various issues and choices facing the new Alliance.
Please complete this on-line questionnaire by August 11, 2010. Your survey will go directly to the external planning consultant helping us with the project, so your replies will be completely anonymous and confidential. If you are not sure of your answer to a particular question, simply leave the item blank.
The survey findings will be used at a Dementia Alliance Board planning retreat to be held in August. You will then have a chance to respond to the emerging plan through a discussion paper, a consultation session to be held in September and discussion at local Board meetings.
