Coming to the Victorian Recorder Guild 2010 Spring Weekend at Myrniong on 1 - 3 October?

You are welcome to provide your registration and medical details using this form. If you have not yet paid, please let us have your payment separately by direct deposit or cheque (details are provided below).

If you would prefer to complete a hard copy form, you can download one from www.vrg.org.au.

Before you start, it would be helpful if you could play through the musical examples circulated with the August VRG newsletter (and available on the VRG website www.vrg.org.au), as there is a question about them further into the survey.

Note that there are several pages to complete - click the Next button on the bottom to go to the next page. The progress bar at the bottom of each page shows how far along you are in the form.

Starred questions need at least one answer before you can proceed to the next page.

We need a separate form for each playing participant. If you are coming with non-playing companions (including children) you can use this form for them too. Please make sure you complete a separate medical section for each person in your party. This form provides for up to three companions. If there are more than four in your party, please start a new form when you have finished this one.

NOTE
Early bird prices apply to payments received in full by 31 July 2010.
Concession prices apply to full time students over 16, or people receiving a government pension.
If you have to withdraw, we will refund your payment in full up to 31 August, and refund your payment less $75 deposit up to 15 September. No refunds after 15 September.
