1. Hodia summer camp - Staff Survey

 
Thank you for taking a moment to share some feedback about camp. All comments are used to improve camp programs in the years to come. We hope to see you soon!

1. We are interested in how we performed communicating prior to camp. All of our communications are typically done by e-mail prior to camp - with very few phone calls and no "snail" mail. Was this effective for you?

2. Program evaluation: Did you enjoy the daytime activities at summer camp? (Depending on which summer camp you attended, these activities may have included horseback riding, white water rafting, hiking, riverfront activities, arts and crafts, etc.)

3. Program Evaluation: Did you enjoy the other evening activities offered at camp? (Depending on the camp attended, this may have included a "prom", relay races, a "carnival", a trip to the Blaine Aquatic Center and BBQ, hang-out time/dance, etc.)

4. Was the structured and non-structured diabetes educational and emotional support experience at Hodia summer camp sufficient to meet the needs of the campers?

8. Did you feel like your role at summer camp was fulfilling to you personally?

10. Was the program structured in a way that campers learned more about diabetes during camp?

12. Are you interested in a revised role at camp? Please be specific and let us know what strengths you might have that may be underutilized or maybe not known to senior camp staff. We would love to help you develop any skills you might have.

15. Director evaluation - How would you rate Don's performance as Camp Director? This performance evaluation should include consideration of camp planning, camp execution, program planning, supervision of camp activities and leadership before, during and after camp. Positive feedback is always enjoyed - and constructive comments and/or criticism are also welcome. We use all comments to improve camp planning and execution.

16. Assistant Camp Director evaluation - How would you rate Lynn's performance as Assistant Camp Director? This performance evaluation should include consideration of camp planning, camp execution, program planning, supervision of camp activities and leadership before, during and after camp.
