Dear BRAiN applicant:

This form initiates your application for a BRAiN Undergraduate Scholar award. Other materials (transcript, statements, interviews) are necessary to complete the application process.

Please enter the requested information about your academic efforts and research interests. Please be sure to respond to all questions.

We use this information to make recommendations for program placement and for reporting purposes to our funding agency, the NIH.

Please note, as per NIH policy, BRAiN can only support students who are a U.S. citizen, a non-citizen national, or a permanent resident. In addition, as per UCD policies, a background check may be required.

Award offers are contingent on a) submission of official documents proving you meet NIH and NMSU eligibility requirements (see above), and b) availability of funds from NIH.

1. BRAiN students must meet the following NIH funding agency eligibility requirements to receive salary/wages support from the BRAiN program:

1- Students must be U.S. citizens or non-citizen nationals or permanent residents; and

2- Students must be matriculated full-time in baccalaureate programs in biomedical or behavioral science fields at the applicant institution

3- As per federal agency requirements, BRAiN applicants who seek to earn salary from this grant must meet the following federal agency eligibility criteria described at the grant URL: http://grants.nih.gov/grants/guide/rfa‐files/RFA‐MH‐10‐070.html

"Eligible individuals are those who are considered underrepresented in the fields of biomedical or behavioral sciences, such as:
a) individuals from underrepresented racial and ethnic groups;
b) individuals with disabilities; and
c) individuals from socially, culturally, economically, or educationally disadvantaged backgrounds that have inhibited their ability to pursue a career in health‐related research.”

2. BRAiN students must meet the following programmatic eligibility requirements to receive salary/wages support from the BRAiN program:

1- Interest in a career in neuroscience.

2- Interest in furthering career through graduate studies after completion of the BS or BA degree.

3- Must be matriculated full-time in baccalaureate program in a science, psychology, mathematics, or engineering major at UCD.

4 - Able to work 15hrs/week during Spring and Fall 2013.

5- Able to participate in paid summer research experience for 40 hours/week at UC Denver School of Medicine during summer 2013.

6- Able to attend national conferences in neuroscience (expenses paid).

7- Completion of at least 9 credits of science, math or engineering courses at the time of the application.

3. Students accepted as BRAiN scholars are expected to be prompt and participate to the best of their ability in all activities every day for the duration of the internship.
