Forces of Change Assessment is one of four assessments in the MAPP (Mobilizing for Action through Planning and Partnership, a NACCHO tool). This assessment is aimed at identifying forces –such as trends, factors, or events- that or are influencing the health or quality of life of our communities or the work of the local public health system.

5. HEALTHCARE (access implies both availability to care and ability to get to provider)

8. What, if anything, should be included in the State Health plan to address your concerns?

9. Which of the identified key health issues are the most important to our community? And why?

10. Maine has made significant strides in tobacco use reduction if we could work on one other public health issue what would it be? And why?
