The ADHS-BWCH is in the process of developing the 2013 Application for the Maternal Child Health Block Grant, which funds a majority of our work, and we need your help. The Application describes how the Bureau works to improve the health of women and children through use of National and State Performance Measures. The 2010 Arizona Title V Maternal Child Health Needs Assessment guides us in establishing our performance measures.

We would like your feedback and to hear your ideas, comments or concerns about how we are doing, what is working well and thoughts or suggestions on how we might do better. This survey will take approximately 5 minutes to complete.

Please pass this information on to anyone who may be interested in the well-being of women and children, including children and youth with special health care needs, in our State.
