Unified Improvement Planning: District Feedback/Evaluation Form
Please provide information about implementation of the Educational Accountability Act (SB-163) and Unified Improvement planning. Your participation in this survey is optional and all responses will be confidential. You have been selected to participate because you are the accountability contact in your district or because your school has completed a significant portion of an entire planning cycle (e.g., submitted a school plan in January 2011). Your response will help to shape the adjustments that the state makes in the Unified Improvement Plan (UIP) process for the 2011-12 school year. The main parts of the survey are: (1) The UIP Template (for districts who used the template this year)(2) UIP Feedback from the Colorado Department of Education (for districts that received feedback), and (3) Training and Support. You will only be asked to respond to questions that pertain to your experience. The survey should take 10-15 minutes.
