Thought activities relating to exercise

The purpose of this form is to give you information about the research study. You are being invited to take part in a study about participation in exercise and your feelings towards exercise.

In this study “exercise” includes any physical activity which you take part in currently or would like to take part in. This includes walking, jogging, going to the gym, and taking part in individual or team sports.

What will I be required to do?
You will be asked to complete 3 questionnaires which ask you to record how much exercise you do and how you feel about exercise. These questionnaires can be completed online or on a paper-version. You may also be asked to complete an exercise plan and/or to imagine and write down the steps you need to take to take part in a physical exercise or activity.

You will be asked to complete these questionnaires now and then again in 4 weeks and in 6 weeks’ time.

You do not have to take part in any physical exercise or activity as part of this study. If you have, or think you have, a medical condition you should consult your doctor before starting any exercise program.

Where will this take place?
The questionnaires can be completed online on a computer or in a paper version.

How often will I have to take part, and for how long?
You will be asked to complete questionnaires three times: now, in 4 week time, and in 6 weeks’ time. The total time to complete the questionnaires will be approximately 15-20 minutes.

When will I have the opportunity to discuss my participation?
Before you start this study you should ask any questions you have to the investigator, whose contact details are provided at the end of this sheet. You will also be provided with the contact details of the investigators who can be contacted at any point throughout this study. At the end of the study you will be informed by email about the results of this study.

How will my data be used?
Only the researchers directly related to this project will have access to your data. All information will be stored in a secure location including password-protected electronic files. The data will not be passed to other people or used in any study other than this one.

The findings from this study will be presented to staff and post-graduate students at Sheffield Hallam University. The findings will also be written as a journal article and may be submitted for publication to a relevant, peer-reviewed journal. The findings may also be re-produced for conference presentations. All information that you provide will be reported in a way that protects your privacy by avoiding any mention of your name or any specific information that could identify you.

What if I do not wish to take part?
Your decision to take part in this research study is completely voluntary. There will not be any penalty or loss of benefits to you if you decide not to take part or withdraw from this study.

What if I change my mind during the study?
You may withdraw from the study at any time..
