The information from this assessment will help us better understand your business and structure your plan to attain your goal of a net profit increase of 15% in 24 months.

The information from this survey is safely stored and protected. Only Fast Forward Process staff and coaches have access to this data. Some required summary data is provided to funding agencies but individual data is never reported. Some data is collected for research purposes.

The information is initially used in the selection process to determine if your business is a good candidate for this program. The assessment will take about 40 minutes to complete. If you need to leave this assessment and come back to it later, please do so using the same computer in the same location and you will be taken back to where you left off.

It is necessary to complete this assessment before participating in a Fast Forward Process Workshop.

An additional short questionnaire on the cost of doing business will be sent to you at a later date. This data will be used for research purposes.
