This is an online registration form for the FPWR Canada National Conference and Gala. The weekend will start Friday with a Gala to raise funds and awareness for Prader-Willi Syndrome. The National Conference will have a Therapy Day which will address some therapeutic approaches to provide early intervention and recreational fun. The Medical Research Day provide families with the latest updates on PWS Research and Medical Best Practices and provide a fun day for Children and Siblings to socialize and learn about science. Here is the key information:

Cost for 2-day Conference: $75 per person

SATURDAY:
All participants will receive a nutritious lunch and refreshments
All activities will be free but spaces are limited

Parking at Bloorview is $7, however, free street parking is available on nearby side streets.

SUNDAY:
All participants will receive a nutritious lunch and refreshments
Free conference handbook filled with valuable information
Free children's program along with supervision for all children and siblings
Free admission for children to Science Centre

Free admission for chaperones (if you wish to bring one)
