1. Consent
You have been asked to participate in a study that is collecting information about Local Educational Foundations. The survey is collecting data to determine how foundations are functioning and the roles that they play with the school district that they serve. The questionnaire is being supported by the National School Foundation Association and the Executive Director, Jim Collogan as well as National University.
The investigators for this study are Donna Elder, Ed.D., assistant professor at National University and Wayne Padover, PhD., associate professor at National University. They may be contacted via phone at 310-662-2142 or via email at delder@nu.edu or wpadover@nu.edu.
The main purpose of this form is to provide information that may affect your decision about whether or not you want to participate in this research project. By deciding to participate in answering the questionnaire, this will be understood as your consent to participate in the study.
Participation in this study will require about 30 minutes to complete the questionnaire. There are no risks if you decide to participate in the study.
There is no compensation or other benefits for you as a participant in the study.
If there are any changes to the study, you will be notified.
All information will remain confidential. All of the data collected will be reported in an aggregated manner that will not identify any participant.
Participation in this study is voluntary. At anytime you may decide not to continue.
In order to comply with federal regulations of secured data storage, National University Institutional Review Board Policy requires that records shall be retained for at least 3 years, and records relating to research which is conducted shall be retained for at least 3 years after completion of the research. All records shall be accessible for inspection and copying by authorized representatives of the department or agency at reasonable times and in a reasonable manner.
By proceeding to the survey, you are indicating (1) that you have read this form or have had it read to you and (2) that you understand this form, the research study, and its risks and benefits.
