Dear Parents/Guardians,
The District of Thunder Bay Social Services Administration Board is seeking to understand your preferences and needs for child care.
Typically, licensed child care centres provide care between the hours of 6:45 a.m. to 6:00 p.m. Monday through Friday. At the present time, extended hours or evening care is only offered at a limited number of programs. The feedback you provide will assist us in determining the need for expanding capacity within the licensed child care system for extended hours and overnight care.
This survey should take only 5-10 minutes to complete. All survey participants that choose to leave their contact information will be entered into a draw for a chance to win 1 of 3 $50.00 gift cards from the SuperStore.
Your responses are confidential and for the use of TBDSSAB’s Children’s Services Division only. No personally identifiable information will be shared or publicized. TBDSSAB’s use and sharing of personal information is regulated by the Municipal Freedom of Information and Protection of Privacy Act.
