Continuing Education Reqirements

Follow the instructions below to obtain continuing education credit for programs that you attended.
1. Attendance and participation in all programs are required. You must not miss more than 10 minutes of any program.
2. Complete the attendance form distributed at registration check-in and place the white and pink portions of the form in one of the drop boxes located throughout the hotel prior to leaving the convention. Keep the yellow copy for records. If you forget drop the attendance form in a drop box, you may mail it to MPA. However, a $10 processing fee will be assessed if received after March 5, 2010. Forms submitted after April 2, 2010, will not be accepted or valid.
3. Complete online posttests and evaluations for each program by completing the following steps:
A. Go to MichiganPharmacists.org/education/continuingEducation/live.
B. Click on the program or event that you attended.
C. For knowledge-based programs, complete the posttest and evaluation for each program attended by March 15, 2010.
D. For application-based programs, complete the evaluation only for each program attended by March 15, 2010. Completing posttests for application-based activities is not required. For knowledge-based programs in which presenters did not submit posttest questions, attendees will not be penalized.
If you are unable to complete posttests and evaluations online, contact MPA before March 5, 2010, to request hard copies.
Online evaluations and posttests will be open for completion until March 15, 2010. If all of the above requirements are met, you will receive a statement of credit in approximately eight to 10 weeks after the convention.
