Online survey of teachers who previously participated in the program

INSTRUCTIONS

This survey is part of an independent evaluation of the RACV Transmission Program. The responses from this survey will be used to improve the program. The survey should take between 5-10 minutes to complete. Your responses are anonymous and confidential, and you will not be identified in any way in reports to RACV. Your assistance with completing this questionnaire is much appreciated.

1. What year(s) have you taken part in the program?
2. Have you ever seen an RACV Transmission commercial on television?

3. What year level(s) and learning or study areas have you incorporated the RACV Transmission program into?
