Welcome to our camp survey. We encourage you to complete the survey in its entirety. It should only take 5-10 minutes. If you complete all of the required questions and provide us with your contact information at the end of the survey, you will be entered into a raffle that will be worth 1 week of camp, free of charge.

We will pick 1 winner at the end of the second Operation Purple camp and that winner will be able to return this summer for one week of camp free of charge.

We will pick another winner at the end of the summer from the rest of our campers survey's and that winner will be able to return in the summer of 2011 for one week of camp free of charge.

This is our way of both encouraging you to participate in this particular survey and thanking you for doing so.

Please discuss the survey with your camper and fill it out with them soon after their experience while it is still fresh in your minds.

We prefer that you complete one survey for each camper for each session of camp attended. This will increase your chances of winning the drawing and give us complete information on each session.
