Congratulations! You’re about to give your business a checkup. After answering the questions below, you’ll receive a report that will allow you to assess the strengths and weaknesses of your business. As the CEO, owner, general manager or key executive, your satisfaction with the issues drives this report. Please note that the specific information that you provide will be kept confidential and will not be used in a sales capacity so please be honest.

Here are a few things to keep in mind:
• It’s a self-diagnostic tool, and it is up to you whether or not you share the results with anyone.
• If you’re struggling with an answer, go with your “gut" or choose "neutral".
• Please answer the questions independently. Several questions are about you, as the leader of your organization.
