The Integrated Falls Prevention Framework: How do we go about implementing it in our region?

Developed by a multi-sector provincial working group co-led by the LHINs and public health, the Provincial Integrated Falls Prevention Framework will guide the LHINs, public health units (PHUs) and their partners in effectively reducing falls and the impact of falls on seniors in a collective and integrative manner.

Many Ontario LHINs, PHUs and health service providers (HSPs) are delivering falls prevention initiatives; however, the specific purpose of the Framework is to integrate and coordinate these initiatives, across the continuum of care and across the province.

The following two approaches are outlined as part of the framework:

1) An effective LHIN-wide Integrated Falls Prevention Program in each LHIN and

2) A Provincial Structure Responsible for Collaboration and Alignment at a Provincial and National Level

This collaboration between the LHINs and Public Health at the provincial level is the first of its kind. It is anticipated that this will foster and lead to further collaborations between the two sectors to achieve better health care outcomes.

We are delighted to bring you the second webinar in the series. The presenters have not been finalized as of the time of this posting. This will be added as soon as they have been confirmed.

However, the focus of the second webinar will be to discuss the regional implementation of the Integrated Falls Prevention Framework and share ideas, suggestions and plans to move it forward.

This knowledge exchange session will use Adobe Acrobat Connect, an internet application, and a concurrent teleconference. Registration for this session will close February 20th at 5:00 p.m. OR at 250 registrants (whichever comes first), meaning you should register as soon as possible, and if you become unavailable, let us know so we can let others register in your place. You will receive a confirmation email on February 20th or 21st, with the website link and teleconference information you need to participate in the webinar.
