Hospice Social Workers and Life Review Intervention Informed Consent and Survey Instructions

 
The survey that you are being asked to complete was designed to collect information about the experiences of hospice and palliative care social workers with life review/reminiscence/legacy activities and the extent to which they are implemented in hospice agencies. You are being asked to participate in the survey because you are a hospice social worker or a palliative care social worker.

Life review and reminiscence interventions have been shown to be beneficial to both individuals facing the end of life and their caregivers by reducing stress and depressive symptoms. The information you share may help in the design of programs that can make this intervention more widely available to individuals and families who could potentially benefit from it.

This online administration of the survey ensures your anonymity and confidentiality. The researcher will not have access to identifying information (such as names, email addresses) for the respondents. The researcher will only receive de-identified data in aggregate form from the online survey administrator for the purpose of data analysis.

Participation in this study is voluntary and you may discontinue participation at any time. Completion of the survey should take about 15 minutes.
