I agree to participate in the research project titled "Norfolk State University Study Abroad Survey" being conducted by the International programs office here at Norfolk State University. I have been informed that the purpose of the study is to find out NSU students interest in study abroad programs.

I am aware that my participation is voluntary and may be withdrawn at any time without penalty or prejudice, and that if I have any additional questions concerning this study, I may contact C. Lakendra Brunston-Parker at (757) 832-2819.

I understand that the intended benefits of this study include marketing research for the International Programs Department, and that there are no risks as a result of this study. I understand that all information is kept confidential.
