1. Why This Survey?

 
PSW is embracing potential changes in health care delivery based on the new health care reform law. Quality of care seems to be a dominant theme which can only be reported through measurable metrics. We are trying to determine current capabilities and interests among our PCPs. This is designed to be a 2-3 minute survey. Please return by Friday August 6th. Thank you.

1. Please share contact information with us. We need to know the best email and phone contact for our updated registry. Give us the most useful contacts; personal email, practice or administrator. We hope this will limit the fax volume and be more efficient. We will only send important communication. Thank you.
