Incidence of Harm & Adverse Event Reporting
 

 
Reports of incidences of harm and adverse events are key to demonstrating the need for a practice act for LDNs in Pennsylvania in order to better protect the citizens of Pennsylvania. The reports are important evidence to illustrate to the legislators the need for a practice act.

Your reports are necessary for the practice act efforts. Simply tell the story of what happened including the advice given, the product used or advice followed by the client, and potential or actual negative outcomes. Actual and potential negative outcomes include, but are not limited to: adverse health events and impacts, provision of inaccurate or misleading information, financial impact, (including physician visits, hospitalizations, and product/consultation costs).

Document the incident in detail using the survey below. Providing your contact information will allow the PADA to gain additional information as needed and acquire a group of registered dietitians willing to share their stories with legislators in the future.
