INVITATION
You are invited to participate in a study that involves research. The purpose of this study is to examine coaches’ confidence in potentially dealing with atheltes that may be taking banned substnces..

WHAT’S INVOLVED
As a participant, you will be asked to complete 2 surveys on your coaching behaviors and attitudes. Participation will take approximately 15 miunutes of your time.

POTENTIAL BENEFITS AND RISKS
Possible benefits of participation include having a better appreciation of your own coaching behavior. There are no known risks associated with participation in this study.

CONFIDENTIALITY
All information you provide is considered anonymous; your name will not be included or, in any other way, associated with the data collected in the study. Furthermore, because our interest is in the average responses of the entire group of participants, you will not be identified individually in any way in written reports of this research.
Surveymonkey stores data in the U.S. and is subject to the Patriot Act.
Data collected during this study will be stored in a secure location on Brock University campus. Data will be kept for 3 years after which time the surveys will be shredded. Access to this data will be restricted to the investigator.

VOLUNTARY PARTICIPATION
Participation in this study is voluntary. If at any time during the survey you wish to discontinue participating, simply stop entering information and leave the website. Your data will not be saved.

PUBLICATION OF RESULTS
Results of this study may be published in professional journals and presented at conferences. Feedback about this study will be available through Dr. Philip Sullivan at the phone number and email address given above. Feedback should be available within 6 months or your participation.
