17-B JCPC Youth Survey
 

1. Informed Consent

 
The purpose of this survey is to collect youth opinions and perceptions about the prevalence of criminal gangs, gang activity and gang membership in Surry and/or Stokes Counties, North Carolina. We are attempting to collect as many paper and online survey responses as possible from youth within our community.

Findings of this survey will be used by the Surry and Stokes Counties Juvenile Crime Prevention Councils to prepare a published report of current criminal gang activity within Judicial District 17-B. Survey comments will also be used to help design local criminal gang prevention and intervention programs as well as apply for additional funding to address the needs of at risk youth in each county. Additional information about the project as well the survey form in .pdf and online versions, are available at our website www.seventeenb.org.

The survey will be available online on April 15, 2010. The deadline for online and paper survey responses will be May 15, 2010. Completed paper surveys may be mailed to the name and address below. We will neither report individual survey responses nor attribute any data to a specific respondent without written permission of the person submitting the information. All survey responses are confidential and anonymous and Internet Protocol (IP) Addresses are not being tracked. Survey findings and aggregated data across projects and centers will be reported through our website, via interactive data displays, and through formal reports to the Juvenile Crime Prevention Councils of Surry and Stokes Counties.
