• This survey is reopened for those who are already receiving a pension from Delphi, or from a Delphi related company, and have received a specific Pension Reduction Notice from the PBGC since 2/20/2010.
• Please input your data WHEN YOU RECEIVE YOUR PENSION REDUCTION NOTICE. If you already provided your Pension Reduction information to the previous survey please do not enter it again.
• One complete response is allowed per computer. Once you complete the survey and hit "Done" at the end you will not be able to return to change responses.
• Your input is totally anonymous, there is no option for you to provide any identifying or contact information, and no way for your information to be related to you.
• This survey will remain open through the period that the PBGC is issuing Pension Reduction Notices.
