T1+ Employee return-to-work following long-term sickness absence - BOHRF

 

This questionnaire is part of a project funded by the British Occupational Health Research Foundation in collaboration with Goldsmiths, University of London. We are exploring the role of the line manager in the return-to-work process. In particular we are examining the various behaviours a line manager can display that aid or hinder the successful return-to-work of employees who have suffered from long term sickness absence (4 weeks or more continuous absence).

We are particularly interested in people who have recently returned to work or attempted to return to work following long term sickness absence due to anxiety and depression, back pain, heart disease or cancer.

This questionnaire asks a number of questions about you and your illness, your well-being, your line manager's behaviours while you were absent and when you returned and finally what impact your illness has had on your work. We are keen to understand what works and what needs improving so that we can develop comprehensive guidance to support others returning to work. The questionnaire should take you 30 minutes to complete.

All information you provide in this questionnaire will be anonymous and completely confidential. All information will be held by Goldsmiths, University of London, will be used only for the purposes of research, and its storage and usage will conform to the requirements of the Data Protection Act (1998).


Taking part in this study is voluntary. You are under no obligation to complete this questionnaire, and are free to withdraw from the study at any point until you return the questionnaire to us.

Should the questionnaire raise any concerns which you would like to discuss, you may talk in confidence with your GP or your local support group.
