Dear Illinois Respite Provider,

Thank you for taking a few minutes to complete this survey. The Illinois Respite Coalition is responsible to implement a federal Lifespan Respite Grant funded by the US Administration on Aging. The grant will allow us to improve accessibility, affordability and quality of respite in Illinois.

As part of this grant, we are collecting data on the many wonderful respite programs available to caregivers throughout Illinois. Please take just a few moments to complete this survey about your respite services. We appreciate your time to ensure that families have accurate information and access to respite resources.

In the future, you will see additional surveys and we hope you will continue to support our efforts by filling them out. This information will ensure that respite care continues to be provided to the many families that depend on it across the lifespan, regardless of age or disability.
