Risk factors can increase a child's chances for drug abuse, while protective factors can reduce the risk. Family, school, community and peers all play an important role in reducing this risk.

This survey will help determine what risk and protective factors exist for your child.

This is an anonymous survey.

1. My family eats dinner together three or more times per week.

2. My child attends church, synagogue or other faith programs.

3. Our school sets clear rules and standards about using drugs or alcohol.

4. I talk to my child about the harmful effects of using tobacco, alcohol or other drugs.

5. My child's school teaches about the harmful effects of tobacco, alcohol or other drugs.

6. I know my child's peer group.

7. I know the parents of my child's peer group.

8. My child has positive relationships with extended family members.

9. My child participates in community activities such as scouts, recreation teams, and youth clubs.

10. My child has good connections with teachers.

11. I know the parents of my child's friends well enough to contact them.

12. My child's friends know the risks and refrain from alcohol or marijuana use.

13. My child faces consequences/punishments when s/he breaks the rules.

14. I talk to my child about sex and other sensitive subjects.

15. My child likes school and wants to succeed.
