Thank you for registering for the 2010 Dental Health Forum, to be held Tuesday, September 14th at the Maryland Dental Association in Columbia Maryland.

There is no fee to attend this forum, however, if your program may need assistance with travel costs (mileage, hotel, etc.) in order for you to attend, a limited number of scholarships are available. Please indicate if you are applying for one of these at the end of this form.

You must use this form for EACH member of your staff who is registering. For multiple attendees from one program, reenter the form after you have completed the first registration.
