Louisiana Medical Home Population Survey 2010 - Louisiana Health Care Quality Forum
1. Patient Centered Medical Home - Louisiana 2010 Population
Instructions
The Louisiana Health Care Quality Forum’s (Quality Forum) mission is to lead evidence-based, collaborative initiatives to improve the health of the people of Louisiana. The Quality Forum was incorporated as a private nonprofit organization in July of 2007, and in January of 2008 was designated by the Agency for Healthcare Research and Quality (AHRQ) as one of the nation’s first 14 Chartered Value Exchanges. The Medical Home Committee of the Quality Forum was formed to promote the use of patient centered primary care in Louisiana as the foundation of coordinated, quality-driven health care.
At present, Louisiana has 53 NCQA certified patient centered medical homes in the state. This is more than any other state in the country and we want to continue to be the trendsetter. The 2010 goals of the Medical Home Committee are to increase the awareness of and population being served by PCMHs, establish peer to peer mentoring program to support PCMH transformation, and promote payment reform to support PCMH. The LHCQF is conducting this survey on behalf of our Medical Home Committee to assess the population currently being served by patient centered medical homes in the state of Louisiana.
Please assist us in this effort. We ask that you take a few minutes to complete this survey to help us advance the patient centered medical home process in Louisiana.
Someone who is familiar with the characteristics of the health care facility, including the types of patients seen, the services provided, the insurance type billed, and the NCQA facility certification level achieved should complete the survey. We appreciate your response.
Thank you,
Cindy Munn
