NHLC Summer Program Survey

1. NHLC Summer Program 2010 Survey

We were delighted to offer the Day Program, Half-Day Program and Specialty Camps to members during the 2010 season. Our goal is to replicate our successes and improve our program based on member feedback about this pilot program. Please fill out this survey to give us feedback on your child's experience at NHLC Summer Program or potential enrollment in future years.

If the question is not applicable to your child's experience, please leave it blank and move on.
