Dear Students,

The Chartwells foodservice department for the Plainfield school district is conducting a survey to understand your likes and dislikes with the school food service. Questions will cover food, services and dining area atmosphere. Your opinions are important. Please complete this survey whether you eat or do not eat food served by the school foodservice.

We are striving to meet the wants & needs of the students and provide the best foodservice program possible. This survey will help us determine students' perceptions regarding the quality of food and services in our school foodservice program.
