Parent Survey: Care of Children and Adolescents with Diabetes Mellitus While in School and School Associated Activities

 
1. Purpose of the Survey; You are being asked to participate in an online survey concerning your personal experiences with the care of your child (parent) or caring for children and adolescents with Type 1 Diabetes Mellitus (T1DM) while they are in school or participating in school sponsored activities. The purpose of the research is to survey children and adolescents with T1DM and their parents. From these surveys we hope to identify persistent barriers to proper diabetes management of children/adolescents with T1DM while they are at school.

Procedures to be followed; You will be asked to compete an electronic survey concerning your personal experiences dealing with your child with T1DM while they are in school or your personal experiences caring for such a child. If you are the parent of a child completing the survey you may assist your child filling out their survey and complete the parent survey as well. The survey will be confidential and anonymous.

Duration of Survey; This will be a one-time occurrence.

Specific procedures that are experimental; The collection of this information is for research purposes to help us identify persistent barriers to the care of children/adolescents with T1DM as well as care plans and school polices which are effective.

Risks and Discomforts; The data collected in this study will be from a survey and is totally confidential and voluntarily. No personal identifying information is collected the risk to the participants is minimal. We are asking your zip code address to look at different regions of the country to see if there are differences. The content of the survey may increase a person’s awareness of barriers contributing to the appropriate management of children with T1DM in schools which may be distressing for some.
