Perkiomen Valley Aquatic Club - 2009-2010 Parent Survey

 

Thank you for taking the time to complete this survey. This is your opportunity to provide the PVAC Board and Coaching Staff with valuable feedback on our program and club. We will review your responses and take them forward as we plan for the 2010-2011 season.

PLEASE COMPLETE ONLY ONE SURVEY PER PARENT. In the past, there has been one survey per/household, but we realize that both parents may want to provide feedback. To complete the survey, simply indicate your responses by clicking the appropriate box or button for each question. There are also numerous opportunities throughout the survey to provide comments and additional feedback on specific questions.
