PTR Membership Survey

Please take a moment to complete your Member Survey below. This provides us with valuable information in order to improve your membership benefits and services.

In November/December, the Membership Survey is required to apply for a Team HEAD exclusive-use Racquet Contract (for US Members Only). Team HEAD contracts are selected each year in January/February and distributed at the Team HEAD Party during the PTR International Tennis Symposium. This year, since the Symposium is in May, they will be distributed in March. Even if you were previously on Team HEAD, you must apply each winter by completing your survey and sending in documentation of how you fulfilled your contract from the previous year.

Team HEAD applications are being accepted for 2013 at this time.
