The California Academy of Family Physicians has identified helping our members learn about and implement the components of the Patient Centered Medical Home (PCMH) as a top strategic priority for the next three years. The patient centered medical home typically focuses on the following areas: clinical decision support; delivery system design (e.g., team meetings, care management); health information technology; patient self-management; team-based care; and quality improvement.

To help guide the development of activities, tools and resources, we are requesting that you complete the following survey. The survey includes questions to help us understand your views and perspectives on PCMH, as well as what would be most helpful to you.
