Thank you for taking the time to answer some questions about the programs and services that you receive from Planned Parenthood Toronto (PPT). We want to know what you think about how we are doing and what we could do better. This survey should take 10-15 minutes to complete.
The survey is completely anonymous and confidential. Participation in this survey is voluntary. This means that if you decide that you do not want to complete this survey, it will not affect your ability to use PPT’s programs and services.
This survey is intended for people who have used the programs or services of PPT within the last year.
After completing the survey, you will have the opportunity to enter a draw for a $100.00 Visa gift card. If you choose to enter the draw you will need to provide us with either your email address OR your first name and phone number so that we can reach you. This information will be stored separately from your survey responses. Your answers to the survey questions will not have any effect on your eligibility to be entered into the draw.
