1. This survey seeks information regarding Polycystic Liver Disease. The researcher, Dr. Wendy Turner-Frey, is an associate professor in the Department of Social Work at the University of Southern Indiana. She is gathering information from persons diagnosed with this disease in order to gain better understanding of symptoms and illnesses that may be related to PLD, as well as patients' subjective experiences with the disease.

Your participation in this study is completely voluntary, and you will not be provided with compensation for completing the survey. You may stop taking the survey at any point in time.

The survey should take approximately fifteen minutes for you to complete. The survey is completely anonymous, and the researcher will have no way of identifying persons who have completed this survey.
