Dear Colleague,

The Pulmonary Hypertension Care Center (PHCC) initiative has been a priority of the Pulmonary Hypertension Association and its Scientific Leadership Council since 2012. The PHCC Committee has been devising an implementation plan for the program. Given its national scope and the many impacted parties, the PHCC program will be an incredibly complex and intricate undertaking that is unprecedented in our field. As such, the program will first be introduced through a small pilot program involving a few sites in early 2014, before its full implementation later next year.

A significant challenge for the PHCC Committee has been forecasting the number of potential applications that will be submitted. The number of applications and potential accredited Centers will heavily influence the length of the roll-out period, the program’s budget, and the size of the Review Committee. The PHCC Committee requests your help by completing this brief survey, indicating your likelihood for submitting an application and what, if any, obstacles your institution might face.

In order to answer the questions and to learn more about the PHCC initiative, including a complete list of Center criteria and the accreditation process, please visit www.phassociation.org/PHCareCenters/MedicalProfessionals/CenterCriteria. Please keep in mind that the PHA is projecting reasonable accreditation and re-accreditation fees in order to off-set some of the costs for conducting site visits and maintaining the program, which is customary for most other accreditation programs. In addition, accredited Centers will be expected, in the future, to contribute to a national PH registry that will be critical for maintaining the accreditation process and meeting its goal as a long-term quality improvement project.

We appreciate your cooperation and support during this important and challenging next phase of the PHCC program.

Sincerely,
The PHCC Committee
