CONSENT TO PARTICIPATE

 
The University of South Florida is conducting a research study to address the barriers to serving individuals with disabilities at One Stop Centers in Florida. Information obtained through this study will be utilized to develop a comprehensive written report with recommendations and strategies that the Florida Developmental Disabilities Council (FDDC) can undertake to increase and improve employment services at One Stop Centers. In addition, an action plan to implement recommendations through a pilot site program will be developed as part of this initiative.

If you chose to participate in this study, you will be asked to answer a set of questions that was developed by the project advisory committee and approved by FDDC. You will also be given the opportunity to add additional comments at the end of the questionnaire. You may choose not to answer any question. You are not required to participate in this study and there will be no negative consequences to you should you decline. The potential benefits of participation may include service improvements that may have a positive impact. There is minimal risk associated with participation meaning the associated risks are the same as what you face every day. There is no compensation for participation.

Your personal information will not be shared with anyone other than the research team and certain government and university personnel who provide oversight including the Institutional Review Board, the Department of Health and Human Services, and the FDDC. We will not publish your name and no identifying information will be included in the final report to the FDDC. Records and data files will be kept as confidential as possible and they will be destroyed after a mandatory five year retention period. During the retention period, the files will be stored in a locked filing cabinet.

Participation is voluntary and you should not feel pressure to participate.
