ATTENTION Student-Athletes and Teacher-Coaches
If you are interested in being entered into a draw for a chance to win a $50 prepaid Visa/Master Card- Gift Card please make sure to provide us with your name and contact information at the end of the survey. This information is collected solely for the purpose of the gift card draw. The contest is only open to student-athletes and teacher-coaches who attended a 2011/2012 season Championship or Festival and completed the survey.
One gift card will be given away for each season. The draw will take place after the entry deadline.
