This is a ANONYMOUS & VOLUNTARY Youth Sexual Health Survey for individuals 13-24 years of age that contains 35 multiple-choice questions (about 10 -15 minutes). You will be asked questions regarding your sexual health and health-risk behaviors. There are no known physical risks associated with taking this survey; however, you may find some of the questions make you feel uncomfortable or seem offensive. There are no right or wrong answers. You may end your participation in the survey at any time and it will not impact current or future services you receive at any local or state community agency.

Responses from this survey will help community groups and organizations throughout Nevada to better understand what may be contributing to STDs among youth; and, help us enhance or develop new, groundbreaking programs.
