NHS West Cheshire Clinical Commissioning Group is currently reviewing local End of Life Services to ensure we are providing the very best care for our local population.
This questionnaire is about the End of Life Care Services received by you and the person you were caring for in the last months of their life. The information you give will help us improve care for people who are dying, and for their family, carers and/or friends. Your views are therefore important to us, and will inform the redesign of our End of Life Care Services.
We realise this questionnaire may bring back strong memories. If you feel upset or distressed, you do not have to continue with the questionnaire and can stop at any time.

We are interested in finding out the experiences of carers/supporters of all people who have died whether suddenly, after a short illness, or after a long illness – within the last 12 months. We also think it is important to find out about the care you and the family received (as carers) at the time of death and in the months since then. Some of the questions may not be relevant to you, but please fill in as much of the questionnaire as you can.
Your answers to these questions will be treated as strictly confidential. No names will be used in the reports we write.
