1. You are invited to take part in a research study regarding parent perceptions of high school transition services. You were chosen for the study because you are the parent of a child with a disability and your child has exited high school within the last four years.

This form is part of a process called “informed consent” to allow you to understand this study before deciding whether to take part.

This study is being conducted by a researcher named Stephanie Johnson, who is a doctoral student at Walden University.

Background Information:
The purpose of this study is to examine the perceptions of parents regarding the effectiveness of high school transition services for students with significant disabilities.

Procedures:
If you agree to be in this study, you will be asked to:
Complete a questionnaire containing 37 multiple-choice questions stating your perception of the effectiveness of the transition services your child received during his/her high school years.

The questionnaire should take approximately 10-15 minutes to complete.

When you have completed the questionnaire you will be thanked for your participation
and asked to exit the survey site.

Voluntary Nature of the Study:
Your participation in this study is voluntary. This means that everyone will respect your decision of whether or not you want to be in the study.
No one at Walden University will treat you differently if you decide not to be in the study. If you decide to join the study now, you can still change your mind during the study. If you feel stressed during the study you may stop at anytime. You may skip any questions that you feel are too personal.

Risks and Benefits of Being in the Study:
Participation in this study may cause a minimal level of stress, as you are being asked to recall information and services that were provided to your child at least two years ago. The benefits of participation in this study will allow the researcher and other educational institutions to better understand what is missing regarding high school curriculum and transition services for students
with significant disabilities. By gaining a better understanding of the transition needs for high
school aged students with significant disabilities, it could be possible to improve the employment
and adult outcomes for these students.

Compensation:
There is no compensation for participation in this study.

Confidentiality:
Any information you provide will be kept anonymous. The researcher will not use your information for any purposes outside of this research project. Also, the researcher will not include your name or anything else that could identify you in any reports of the study.

Walden University’s approval number for this study is 12-03-09-345527 and it expires on
December 2, 2010.

The researcher will give you a copy of this form to keep (you may print a copy for your records if
you wish).

Statement of Consent:
I have read the above information and I feel I understand the study well enough to make a decision about my involvement. By choosing to participate you are agreeing to the conditions and consent to participate in this study.
