Questions about services for veterans

This survey is for NCHN Network Members and current HRSA Rural Health Network Development Grantees. We are collecting data for the Department of Veterans Affairs. The data will be analyzed and presented at the NCHN Leadership Summit on October 2, 2013.

Summit Session: Rural Veterans and their Healthcare: Results from a Survey of NCHN Members

1.Do you know if clinics in your network serve any Veterans?
2.How many of the clinics in your network would you say serve Veterans? (if unsure, your best guess is ok)
3.Do any of the clinics in your network ever bring up issues specifically related to Veterans’ health care to you?
4.Do clinics in your network exchange patient health care information with any VA facilities?
5.If you answered yes to Question 4:
Do you exchange information electronically through Health Information Exchange (HIE), or do you use fax/mail/hand carry to do so?
6.Do you see any value in Veterans Health Administration programs related to rural health care?
