United Way of Bradley County 2012 Community Needs Assessment

 

The purpose of this project is to gather public opinion to help United Way of Bradley County and the Bradley Memorial Health Endowment Fund to determine which community services are most needed and to help direct resources toward services based on need. Your opinions are important to us and we urge you to join us in our effort to improve the quality of life in Cleveland and Bradley County.

INSTRUCTIONS:
• This is an anonymous questionnaire; you are not asked to identify yourself in any way.
• Complete this questionnaire by February 29, 2012.
• Complete the questionnaire only once.
• We are asking you to rate the entire community’s needs. Near the end of the questionnaire, you are given opportunity to address your personal needs.
